Our Goal

To provide you with an excellent patient-centered experience. In our efforts to serve you better the Ottawa Fertility Center welcomes your feedback/recommendations on how we can achieve exceptional levels of service delivery. Please forward your inquiry to any representative of the OFC Team and feedback will be provided to you within 10 business days.

Patient Complaint / Recommendation

Access any OFC Team Member – for Immediate resolution of concerns.

  • If unresolved, request appropriate Manager/Director contact information from OFC Team Member or send in writing to info@conceive.ca
  • If your matter is about billing, please email billing@conceive.ca
  • If your matter is about privacy, please email privacy@conceive.ca
  • If your matter is about access or making corrections to your records, email accessrecords@conceive.ca
  • All inquiries will be forwarded to the Responsible Manager/Director
  • Responses will be provided within 10 business days (30 days for access to records)
  • If you remain unsatisfied with the response, it will be brought forward to an OFC Internal Committee Structure
  • Communication will occur back to the interested parties
  • Quality Improvements will occur as a result of feedback
  • If the complaint is as a result of an error or potential error, the complaint will proceed through OFC’s Incident Reporting structure & Quality Assurance Committee
